Apart from it being a wonderful location and delicious food, the main reason I’m here is the chance to network with other healthcare facilities. It’s very convenient to come here because we meet with colleagues that have similar challenges, and I can actually bounce ideas off them and find out what works and what doesn’t. If I was to do this from my office, I would probably take many months to have the same level of access to information that I have here.
WorldConnex 2019 Waiver Agreement/Release
- I understand that this Waiver, Release of Liability, Indemnification and Hold Harmless Agreement (hereinafter referred to as the “Agreement”) is intended to address all of the risks of any kind associated with my participation in any aspect of WorldConnex Conference 2019 (hereinafter referred to as the “Event”), including but not limited to, such risks created by actions, inactions, or negligence on the part of GlobeStar Systems Inc./ Connexall USA, Inc.. (hereinafter referred to as “Globestar/Connexall”), its directors, officers, shareholders, servants, agents and employees (hereinafter referred to as the “Releasees”).
- In consideration of being permitted to participate in any way in the Event, I hereby RELEASE, WAIVE, DISCHARGE and HOLD HARMLESS, the Event and the Releasees from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any liability, loss, damage, costs (including attorney’s fees), or injury that may be incurred or sustained to me or my property while or due to participating in the Event, or while in, on, upon, or traveling to and from any program event, optional or otherwise. I further release, waive, discharge and hold harmless the Event and the Releasees from any loss, injury or damage sustained during or as a result of transit, by air or ground, prior to, during or after the Event, whether or not such transit is arranged by the Event or the Releasees.
- I assume all risks, known and unknown, foreseeable and unforeseeable, in any way connected with my participation in the Event, including but not limited to risk of physical injury, damage to property or disability. I voluntarily assume full responsibility for any risks of loss, property damage or personal injury that may be sustained, or any loss or damage to property as a result of my participation in the Event. I further assume all risks associated with my participation in any activity, educational, social, physical or otherwise undertaken as part of or exclusive of the Event.
- I understand that the Releasees will not be responsible for any medical costs associated with any injury.
- I understand that it is my responsibility to abide by all applicable laws during my participation in the Event, including but not limited to laws pertaining to alcohol and alcohol consumption. I assume all risks, known or unknown, foreseeable or unforeseeable, associated with the consumption of food, beverage and/or other products, including alcoholic beverages, whether or not such products are supplied or paid for by the Event or offered at any of the Event’s activities. I further agree to indemnify and hold harmless the Event and Releasees from any loss, damage or injury associated with the consumption of food, beverage and/or other products.
- I also understand that I am urged by the Releasees to obtain adequate health and accident insurance to cover any personal injury which may be sustained during the Event or the transportation to and from the Event and activities related or unrelated to the Event.
- If any term or provision of this Agreement or the application thereof to any person or circumstances shall to any extent or for any reason be invalid or unenforceable, the remainder of this Agreement and the application of such term or provision to persons or circumstances other than those as to which it is held invalid or unenforceable shall not be affected thereby, and each term and provision of the instrument shall be valid and enforced to the fullest extent permitted by law.
- I hereby grant GlobeStar / Connexall the following rights in the interest of furthering GlobeStar/Connexall’s distribution of informational and marketing materials:
- The right to record my image, photograph, picture, and voice by any technology or means.
- The right to copy, use, display, and distribute such recordings of me for any legitimate purpose, including but not limited to distribution by means of streaming or other technologies
via the Internet, or distribution of audio or video files for download by the public. - The right to use my image and voice in connection with the marketing of programs, events, or educational materials.
- I further agree that I will not receive compensation, now or in the future, in connection with GlobeStar/Connexall’s exercise of the rights granted hereunder.
- I hereby assign to GlobeStar/Connexall any and all copyrights (including all other intellectual property rights) I may have in the recordings made of me hereunder.
- I understand and agree that this Agreement shall be construed in accordance with the laws of the Province of Ontario, Canada.
